This is a REGISTRATION FORM to sign up your child as our Autsome Member. All information is required to be completed before you collect your Autsome Kit.

Only children who are diagnosed with Autism Syndrome Disorder (ASD) is applicable to register.

Once this online registration is completed, kindly collect your Autsome Kit at Sunway Putra Mall's Concierge, Ground Floor.

As an Autsome Member, your child may enjoy the privileges (Only applicable on every Tuesday*) below :
- Shopping Privileges
- Priority Lane at participating outlets
- Autsome Parking at B1 (Sticker provided together with Autsome Kit )

Autsome Facilities (Available Daily)
- Calm Rooms (Lower Ground & Level 3)
- Sensory Wall (Level 3)
